We are seeking an experienced nurse who is passionate about providing specialised care to individuals managing chronic conditions. Responsibilities:
  • Develop, implement, and evaluate care plans for patients with chronic diseases, in collaboration with the interdisciplinary healthcare team.
  • Conduct comprehensive assessments of patients’ health conditions and identify individual needs and goals.
  • Provide education and support to patients and their families, empowering them to actively manage their chronic conditions.
  • Monitor and review patients’ progress, making necessary adjustments to care plans as required.
  • Coordinate and communicate with healthcare providers, specialists, and community resources to ensure continuity of care.
  • Utilize electronic medical records and other relevant tools to document patient assessments, interventions, and outcomes accurately.
  • Stay up-to-date with the latest research, best practices, and guidelines related to chronic disease management.
  • Participate in quality improvement initiatives and contribute to the development of policies and protocols.
Qualifications:
  • Registered Nurse (RN) license in Victoria.
  • Bachelor of Science in Nursing (BSN) degree preferred.
  • Minimum 2 years of nursing experience, with a focus on chronic disease management.
  • Strong knowledge of chronic diseases, including but not limited to diabetes, hypertension, heart disease, and respiratory conditions.
  • Excellent assessment, critical thinking, and problem-solving skills.
  • Effective communication and interpersonal skills to engage with patients, families, and the healthcare team.
  • Ability to work independently and collaboratively in a fast-paced environment.
  • Proficiency in using electronic medical records and other relevant healthcare software.
